We have coffee delivered to our cabin every morning! It's not only our wakeup call, but I don't talk to my wife until after she's had at least two cups!! lol
But I'll usually tip $2 for bringing coffee, maybe $3 if we're having pastries or something. At least once during our cruise - usually a day at sea - we'll have breakfast served to us on our balcony . I'll normally tip them $4 - $5 depending on how much we order.
As Char said, usually the room server is not the same as your cabin stewart, so this is somebody you don't see on a regular basis. It's important to provide a cash gratuity to them for their service since they are also not part of the 'tipping pool' should you decide to have your gratuities automatically charged to your cabin.
