Most of the time we skip any excursions and just get into the port and walk around, do a little shopping for our official teeshirt, sightsee a little.
I do a bit like Mindy says. Read some of the standard guidebooks about each port. Go on that city's website (often they have a brochure online or one they will mail to you, maps, too).
A couple of times my hubby has even negotiated with a taxi driver for a local tour -- after getting a sense of the driver's communications skills (one time he wanted to practice his Spanish) and attitude.
We do tend to go with a ship's tour when we want something more planned, and have actually had good experiences with those. Since we don't do this all the time, hubby doesn't mind the rates (in a relative sense).
