I absolutely detest the smell of tobacco (my father was a cigar smoker so was all too familiar with it) but have not had a cabin yet (knock on wood) that smelled of tobacco. I haven't been on Cunard, but from what I understand there's a special spray that can be used, and that it's generally used on ships by the cabin stewards for this purpose.
You would definitely be able to tell when you pass by a major smoker's cabin.
We did have a problem with a Miami hotel years ago. I asked for a non-smoking room. When we opened the door, it positively reeked. We went back to the desk and said this was not acceptable. We were taken up to another floor that had a "no smoking symbol" by the elevator. But when the bellperson opened the door, we were again assaulted by a heavy duty smell which I immediately recognized as "pure cigar." We said no way. The front desk said we would need to wait an hour or so for another room. So we did.
I was on the QM2 in January 07 and had a balcony cabin. There was no smoke smell in the cabin I had. In fact, I've been on 20+ cruises (cruised in everything from insides to balcony cabins) and luckily, have never encountered a cabin that had any smoke smell to it. Guess I have been very lucky.

I do know that the cabin attendants are instructed to sanitize the smell of the cabins so I think in most cases you can't smell the smoke.

We've been pretty lucky, too. In 31 cruises, we've only had one cabin that was really bad. Unfortunately, the ship was sold out, so there were no other cabins available to move us into. So, they replaced the curtains, replaced the bedspread, shampooed the carpet, and sprayed their special stuff on everything. It did the trick.
But this is why most cruise lines are beginning to forbid smoking in cabins now.
Hopefully, you will get lucky, but if not, there are things they will do to insure your comfort.
Here is the smoking policy on Cunard:
For the comfort and convenience of all guests, elevators, and some restaurants and public rooms have been designated as non-smoking. Many public rooms offer both smoking and non-smoking areas. Guests who enjoy pipes or cigars are accommodated in Churchill's Cigar Lounge onboard QM2, and in the Chart Room, Golden Lion Pub, Crystal Bar and open decks onboard QE2. We ask that guests refrain from smoking pipes or cigars in other enclosed public areas of the ship. Aboard QM2, each public room and bar have designated smoking sections and all restaurants are non-smoking. Aboard QE2, each restaurant provides a small section designated to cigarette smoking with the exception of the Princess Grill, which is non-smoking. Each public room and bar have designated smoking sections. The elevators, theaters and library are designated as non-smoking.
We've never had any problems with smoke, except occasionally in the casino.

My wife and I are VERY sensitive to smoke (she is asthmatic, I am highly allergic). On none of our cruises have we detected even the slightest trace of smoke in our rooms. AS for the rest of the ships the only bad places have been the casino and right outside the smoking area of the bar closest to the main restaurant right before dinner.

I have smelt smoke coming from cabins where people are smoking, but I have never been in a cabin which has had a smell left over from the previous occupants.
On QE2 the only places to avoid if you don't like smoke are parts of the Chart Room, the Casino and part of the Yacht Club. I have not been on QM2 or QV but I understand that smoking areas onboard are more limited than on QE2.

When , I had a tour of Queen Mary 2, I noticed that as you walk towards the forward part of the ship, near the commodre club you can really smell the smoke the closer you get.
