We just spend it as we see fit. We may not spend anything at all on one island, but may do tours and maybe have a drink or 2 on another one.....just depends on what we feel like doing.
We don't either. But then we don't go crazy spending as we stay away from the casino and only buy a few drinks (usually hubby). We usually know which excursions we are doing, so we know that much and the minimum for tips (plus extra as needed). The cruiseline doesn't make too much secondary onboard revenue from us.

Lulu hit this one pretty good.
If we had to think about limiting ourselves it would take a lot of the pleasure out of cruising. On the other had we do not spend a lot for extras. If there is something we want to do, we do it. It just happens that there is not a lot that we want to do in most of the ports anymore. We really enjoy ship time.

For my past cruises, I never budgeted to stay within a certain amount, but I had a rough budget of how much excursions would be.
The cruiseline makes alot of money on DH and I with our bar, spa treatments and photo gallery charges. Boyconny, I can't say a word about your wife...I'm the same way! But look at it this way, pictures last longer than those spa treatments!
For the Med Cruise, I had every day budgeted to get a rough idea. If I would have been off, we would have still done what was planned, I had extra budgeted!We did not do any ship excursions and needed to have euros with us since we were doing all of the ports on our own and didn't want to be looking for ATM's.
I use a budget to get a rough idea for planning, but I don't worry about going over.
