An Emergency Announcement came on in our cabin from the CD saying the Captain requested everyone wear low heel, skip resistant footwear. Then, the CD added, "Flip-flops and tennis shoes will be perfectly appropriate for this formal evening."
So I have actually worn flip flops on a Formal Night to the MDR!

On the Brilliance cruise my ankles swelled up really bad (long story, but it had nothing to do with the cruise itself). Later that night, I didn't bother getting changed and put on my Royal Caribbean slippers to head to the Concierge Lounge to use the internet. Well, my card wasn't working in the swiper thing so I had to head down to Guest Relations and the elevator was right outside the CL so off I went wearing my black formal gown with my white RC slippers Yes there were fashion police on patrol, no one said anything, but the looks on their faces were priceless

I would be in big trouble wearing my brown flip flops on a formal night. Then again, if you read what I wrote above, how much worse could it be?

On our 15 day cruise, I brought a total of three pairs of shoes. The walking shoes I was wearing and two pairs of sandals in my luggage. A flat casual pair and a dressy with 1" heels (that's high for me) black sandals.
My hubby also only brought a few pairs of shoes. On our third formal night we both ditched the dress shoes and went casual at least for our feet.

I take flip flops as a backup and never wear them. I can't stand the thing between my toes.
They're lightweight so I keep thinking it's better to have them and not need them; than to need them and not have them
Boy, am I ever with you there. I HATE that thing between my toes. And, I find the 'slapping' noise that they make with every step excruciating irritating.
Back to the original question -
I can't recall havingEVER taken a pair of shoes that I would onlly wear once. I can't even imagine justifying owning enough shoes to have a pair that only goes with one outfit.
