With a woman, I may just throw her a signal (touch my shoulder for a wayward bra strap), etc. or say something quietly to her. I'm usually the one with the peekaboo bra strap.
I wouldn't mention a bra strap. There seems to be a trend today of allowing bra straps to show, so it might be intentional. Open button -- not that important. Open zipper on a man -- I'd be embarrassed to mention it. I would mention trailing toilet paper or an untied shoelace.
