This does not allow for them to sign in case of medical emergency. Here's a site that has some wonderful forms; Travel Consent Form LawDepot.com

Remember if the child is cruising with a friend, BOTH parents have to SIGN IN FRONT OF A NOTERY AFTER PROOF OF WHO THEY ARE, and if separated or divorced, both can sign a form separately, and medical consent is very important Also every child 17 and under needs to have a state ID as well as a passport to sail!!

Some people try to turn back their odometers. Not me! I want people to know why I look this way. I've traveled a long way, and some of the roads weren't paved.
