It may depend on the island, but I'd imagine that locals use those ferries on a daily basis for commuting so they're probably pretty accurate, especially if you're getting the ferry schedule directly from the transportation company for each island.
The only ferries we used on our own in the Caribbean (if it counts as the Caribbean?) were in Bermuda and they, in addition to the bus routes/schedules, were very accurate![]()

Thanks, Judy!
We only times in the Caribbean we have taken a ferry have been on land trips so it was of no real importance to us ... not like missing the ship.
The Red Hook ferry in St.Thomas to St.John was within 15 minutes which isn't bad.
That Speedy one in Tortola to Virgin Gorda was anything but "speedy".![]()
While cruising, there are several inlands where we've considered taking a ferry (on our own) and would like to ... but a little fearful to do so.

I agree LuLu, you just never know if they'll be 100% reliable.
If I could find a back-up Plan B to get me back to the port on time or the ability to catch up to the ship the following day I may give it a try.
I wouldn't have felt that way prior to going on a Med cruise where we did all the ports on our own so I feel more adventurous now![]()

I have been on the ferry between Cozumel and the mainland. It was a miserable trip! The ferry was shallow bottom so it had a lot of motion (up and down side to side back and forth pitch yaw and roll, the works!)
