You will need to check with your cruise line. For some ports, they will include the cost of a visa in the price of an excursion, for others they will insist that you get it yourself, and for others they will let you know what they have worked out with the government.

I don't want to venture to guess what they would do in Indonesia. However, one thing is very clear, if you do not get a visa and they feel you need one, they could charge you a huge fine. The info I found suggests that $25 is for a 30 day visa, and $10 is for a 7 day visa. This may be out of date though.

I recommend you ask your travel agent to research this for you, and to check the Bali embassy website for the latest info.

Check with the Indonesian Embassy for Visa requirements. Do not call the cruise line because they will tell you to check with the Embassy of Indonesia. There is a link for Bali Visa Requirements in the post immediately before this.
